Vaccine Adverse Event Reporting System
VAERS 2.0 includes a new reporting form and a new website that allows you to submit a VAERS report electronically, access VAERS data, and learn about how the CDC and FDA monitor the safety of vaccines. You can use the Online Reporting Tool or complete a VAERS pdf form and upload it to the new website.
